1. https://13.127.15.161/accounts/customer_list/
2.account > customer >sip device and edit sip device
3.observe that username and password field is alignment not properly.

Bug:-
1. account > customer > sip device click on edit sip device.
avilable field is username and password alignment not propely when edit.

Expect:-
alignment should be properly.
